All forms and documents should be emailed to your Academic Dean. Printed forms may also be submitted to the Student Services Office.
There are some very good reasons to complete community service hours while enrolled in high school. As soon as students are promoted to the 9th grade (the month of June following promotion to the 9th grade), they can begin to accrue community service hours towards scholarships. You should know that Florida Bright Futures Scholarships (Academic Scholars and Merit Scholars) requires community service hours to be completed:
- Florida Academic Scholars - 100 Hours
- Florida Medallion Scholars - 75 Hours
- Florida Gold Seal Vocational - 30 Hours
The Community Service Form should be used to document volunteer activities that students participate in at FSUS and in the community.
If available, request the coordinator to document the activity on their letterhead and provide the following:
- Name of student
- Total hours volunteered
- Short summary of what the student participated in and how they assisted
